Legal Services
The legal issues created by aging, illness, chronic conditions or disability can be challenging. This is why every Life Care Plan includes the legal services you need to protect you or your loved one's interest and to provide for dependents and other family members. Depending on you or your loved one's needs and the family circumstances, the Life Care Plan will include one or more of the following legal services:
- Consultations, drafting and implementing a customized and comprehensive Life Care Plan;
- All necessary legal documents such as wills, trusts, powers of attorney, etc.;
- Resolving the asset protection problems created by the high cost of quality long term care;
- Assistance with Medicaid planning and eligibility;
- Assistance with Veterans' Affairs benefits planning and qualification for Veterans' Affairs benefits;
- Establishment of Special Needs Trusts; and
- Probate and estate administration
Care Coordination
Whether you or your loved one is living at home, in an assisted living facility or in a nursing home, you can be comforted in knowing that you or your loved one's care is being coordinated by experienced professionals who have extensive knowledge about the costs, quality and availability of resources in the community. Some of the care coordination services incorporated in a customized Life Care Plan includes:
- Services of an Elder Care Coordinator to assist with the development and implement of the Life Care Plan;
- Initial consultation in your home, residential or health care facility;
- Assistance with locating and hiring in-home help and coordination of available community resources;
- Coordination with medical and health care providers;
- Assistance with living arrangements and placement;
- Provide education, counseling, support, and guidance for you and your loved ones.

We stay by you and your loved one's side working to protect your interests, now and in the future. Some of the advocacy services that will be included in your customized Life Care Plan will include:
- Advocating for quality care in a long term care facility by attending scheduled care conferences to address care issues;
- Communicating with general staff and management of long term care providers to seek resolution and discuss where there are opportunities for improvement;
- Education and empowerment for our clients and their families to help them advocate for good care and recognition of their rights as a consumer of long term care.
