
What to Expect
The Life Care Planning Team at Smith and Condeni LLP uses a model called the Elder Care Continuum to help you and your family understand the natural progression of aging and its impact on you or your loved one’s health, mobility, housing and financial resources. At the initial meeting, we will discuss the Elder Care Continuum and make a preliminary determination of where you or your loved one is on each line of the Continuum.
The Life Care Planning Team will follow up to make a more accurate and calculated Continuum assessment in order to identify gaps in care and suggest options for how to mobilize resources and public benefits to close those gaps and plan for the best care for you or your loved one. When your customized Life Care Plan is developed, it will define, organize, prioritize and mobilize every aspect of you or your loved one’s care, to allow them to maintain the quality of life that they desire and deserve.
To ensure success of your Life Care Plan, we invite the entire family into the process to allow us a better understanding where you or your loved one is situated on the Elder Care Continuum.
